Countertops are usually cast into molds, often the maker will add additives to strengthen and improve the concrete, as well reinforcement (usually steel bars).

How thick and heavy are they?

The standard thickness is 1.5" although 2" slabs common. A 1.5" thick concrete countertop has weight of about 19 pounds a square foot. Standard kitchen cabinets will support a concrete slab due to load balancing.
